A nurse will be assigned as case manager for your care. The nurse will coordinate your doctor’s prescribed plan of care with you and other members of your team to get you the best care possible.

PHYSICAL THERAPY

The physical therapist will help you regain your normal level of function following an illness or injury, or, if not possible, helps you adapt to new ways of moving. Your physical therapists will develop a program that best fit your needs to help you maintain your independence.

OCCUPATIONAL THERAPY

The occupational therapist develops solutions for you to perform daily tasks. These tasks may include getting dressed, using the bathroom, showering or making a meal. Your occupational therapists have great ideas to help you live safely and independently in your home.

SPEECH THERAPY

A speech therapist, helps patients with breathing, swallowing, speaking, cognition and thinking skills and communication disorders. Your speech therapist will perform an evaluation to develop the best plan to help you meet your goals.

MEDICAL SOCIAL WORKER

Medical social workers are available to help determine needs or assistance you may require. They may help you and your family identify available community resources and provide assistance navigating the complexities that are sometimes involved in managing your illness or injury.
